Customer Care Manager We are seeking two highly motivated, experienced Case Managers to oversee a portfolio of customers based in Adelaide. Be part of a high performing team Do work that matters – make a difference in the lives of Elderly Australians Permanent full time position within a growing company Get work life balance – no weekends or night shifts We have a rewarding opportunity for two experienced Case Managers to oversee a portfolio of home care package (HCP) customers based in Adelaide. You will be inspired by making a positive difference to the lives of our customers, conducting in-home assessments to identify their needs and developing tailored care solutions to enhance their quality of life.

Work with flexibility and autonomy, dividing your time on the road visiting our customers and networking with relevant community stakeholders, and liaising with our internal teams and ensure the smooth delivery of care services. Skills and Experience required: Previous experience in case management and working in a community setting preferable Knowledge of the Community, Disability or Aged Care system and care packages desirable Excellent ability to connect and build rapport with multiple stakeholders Team-oriented with an open and collaborative work style High attention to detail and focused on achieving positive client outcomes Resilient and able to work effectively under pressure Empathetic, understanding and passionate about caring for people Advanced computer skills National Registration with Australian Health Practitioner Regulation Agency (AHPRA) desirable but not essential Benefits: Doing work that matters - making a real difference in the lives of Elderly Australians Challenge and variety working with multiple stakeholders and solving complex problems Working in a flexible and supportive team environment within a hybrid home and office arrangement Frequent community engagement Inspired & supportive leadership , have flexibility and autonomy in your day to day Competitive package including a car allowance Permanent full-time position To be considered for this role, you will preferably have a qualification in health and/or community services with strong previous case management experience.
