Some of our staff share their experiences in this short video: https://goo.gl/iShcmS What you&39;ll be doing Funded under the state-wide Pathways to Community Living Initiative (PCLI), this position is responsible for recovery-orientated, person-centred assessment, treatment and care coordination for long stay adult patients transitioning to community based care, along with those at risk of long stay. PCLI is a coordinated state-wide approach to supporting people with enduring and serious mental illness who have been in hospital for more than twelve months and or are likely to experience difficulty with discharge solutions, to re-establish their lives in the community.
Western Sydney Local Health District Mental Health Services are committed to building a professional workforce that supports consumers with a severe and persistent mental illness on their recovery journey. The role of the Clinician is to provide district-level evidence-based care that supports safe transitions of consumers into care in a community setting (or appropriate inpatient setting) based on the individual consumer’s strengths, needs and wishes. These services are to be least restrictive, collaborative, nominated carers and family responsive and inclusive, rehabilitative, encourages self-advocacy, facilitates empowerment and capacity for optimal independent function.
A primary goal of this position is to work collaboratively with staff, consumers, carers and other agencies (both within WSLHD and government and non-government organisations) to promote effective participation in mental health treatment, support and recovery.
