Clinical Navigator Cancer Care - BHSHome Registered Nurse Div1 – Grade 4 Fixed term, full time (01/02/2021 to 01/02/2022) Ballarat Health Services is currently undertaking an expansion of our home-based care, with a newly created opportunity for a Clinical Navigator Cancer Care. This is an exciting opportunity for a Registered Nurse Div 1 to be part of this progressive service. BHSHome is an innovative model of care that will increase the capacity of our home-based teams to deliver safe, appropriate, and person-centred care outside of a hospital setting.

This progressive program, on prevention and early intervention, highlights the prospect of achieving better outcomes for people with complex care needs. The role of the Clinical Navigator Cancer Care is a valuable component in coordinating the delivery and continuity of cancer care services for the patient in their home environment. The enhanced use of programs such virtual monitoring, which form part of BHSHOME, will enable more patients to access medical consultations, systemic anti-cancer therapies (SACT) and supportive therapies in their home, reducing their time and monetary burden, whilst improving workforce capability through support and education.

This is a diverse and complex role, entailing considerable interaction with a wide range of clinicians and health care professionals both internally and externally. The role is required to develop strong working and collaborative relationships with all members of the multidisciplinary team, complimenting and improving outcomes of existing care, systems and services. You will have: Bachelor Degree in Nursing Current registration with Australian Health Practitioner Regulation Agency Demonstrated experience in oncology.

Demonstrated high-level communication, liaison, interpersonal and negotiation skills working with multiple stakeholders Demonstrated ability to provide effective and professional leadership to a multidisciplinary team Relevant post graduate qualifications in cancer care Ballarat Health Services provides a complex range of surgical, medical, obstetric, psychiatric, palliative care, rehabilitation and residential services. Ballarat is located approximately 75 minutes’ drive west of Melbourne and offers a wide range of recreational, sporting and social facilities for a population of over 100,000 people. A vibrant, progressive regional city, Ballarat is large enough to retain its own regional flavour but is within easy commuting distance to Melbourne or the Surf Coast.
