Austin Health is the major provider of tertiary health services, health professional education and research across 3 campuses in the northeast of Melbourne and state-wide. An exciting opportunity exists for an experienced Program Manager and strategic thinker with a passion for integrating acute and community services to join our Strategy, Planning and Integrated Programs team.
The Strategy, Planning and Integrated Programs sits within the Chief Executive Office of Austin Health, under the leadership of the Chief Executive Officer. The team leads health service planning, strategic and business performance monitoring, and contributes to service and program integration at Austin Health. The Development Lead – Integrated Programs is responsible for fostering and supporting collaboration within Austin Health and between local partners to improve the lives of people in our community.
In 2021, the Development Lead – Integrated programs will lead detailed planning for the Patient at Risk of Multiple Hospital Admissions project (PRIMA) – which will see better coordinated and more responsive acute services delivered to at risk patients over the long term. This will involve: - Undertaking a gap analysis and developing a business case and staged implementation plan for a new model of care. - Working closely with the IT team and Business Intelligence Unit to oversee the development and implementation of an automated data-analytics tools to predict patients at risk of multiple hospital admissions.
- Pursuing funding opportunities to support implementation of the project. The Development Lead – Integrated Programs will work to strengthen partnerships with key stakeholders across the community and will also support the broader work of the Strategy, Planning and Integrated Programs team. We would love to hear from you, if you have demonstrated experience in: the development and implementation of contemporary patient-centred models, frameworks and services the transition of strategic priorities into business plans and outcomes the leadership and management of a complex health service / function building collaborative relationships with a varied range of stakeholders, including in acute, community and primary car setting.
