Support Worker - NDIS (Male)

About our organisation Chorus was formed in 2017 by the merge of three established WA care providers, Care Options, Community First International and Volunteer Task Force. We are an Australian not-for-profit organisation providing services in Aged Care, Disability, Mental Health and Volunteering. Together our new organisation will deliver care and services to over 10,000 clients across metropolitan and regional WA with a 480-strong workforce and over 600 volunteers.

The Role We are currently seeking applications for a part-time Support Worker (male only) to support a specific customer by providing tailored person-centred services and supporting them to access activities of their choice. The Support Worker role at Chorus can include: Connecting in the Community and assisting with hobbies and activities where required; Meal preparation; Transportation of Customers to appointments and events; Domestic activities; Personal care. This role will be supporting two young males.

One support will provide support to access the community and teach independence. The customer is looking for support to take him shopping, teach him to use public transport, go fishing (family have gear), to the movies and other activities of interest. The other support will be supporting a customer who is non-verbal.

You will be providing respite to his family. We are looking for one person that can assist both customers at the following times: One full day (7 hours) Saturday once a fortnight between 9am and 3pm Additional shifts with other customers are available to those wanting hours in excess of the above roster. Essential Criteria: Demonstrated ability to work with people with diverse needs and preferences; Minimum Certificate III in Aged Care, Disability, Individual Support or other relevant qualification (or working towards obtaining qualification within 12 months) and/or relevant experience working with customers who are aged or with a disability; Excellent communication skills both written and verbal to interact with a diverse range of customers, their families, carers, community agencies and other professionals; A proactive approach to the safety and well being of yourself and customers; Understanding of professional boundaries and privacy and confidentiality issues; Good computer & mobile device skills.

Prerequisites; National Police Clearance of 6 mon.
