About Department The Health Independence Program (HIP) at SVHM consists of Complex Care Services (CCS), Subacute Ambulatory Services (SACS) and the inner North Melbourne Post-Acute Services (based at North Richmond Community Health (NRCH). HIP offers a comprehensive range of specialist clinics, services and rehabilitation programs, providing rapid response health management, short-medium term care coordination and health education. HIP enable clients to be the decision makers of their care, and support clients through self -management to achieve their healthcare goals.
HIP consists of interdisciplinary teams based at NRCH, SVHM Fitzroy and Kew hospital sites, and care is delivered to clients under a SVHM HIP Community Hub model. Complex Care Services: Psychosocial Team (ALERT) key target groups include patients with complex needs who fall under one or more of the key risk factors; substance use, mental health issues, homelessness, disability and complex aged care issues, family violence. This position is based at St Vincent's Hospital Melbourne within the Emergency Department and also providing support and interventions in the community as part of an interdisciplinary team.
About the Role The ED Care Coordinator through screening and recruitment in the Emergency Department (ED) is responsible for the delivery of timely and comprehensive assessment and care coordination of patients with complex needs to facilitate the allocation of hospital and community resources in an efficient and cost-effective manner. Assessment and care planning are achieved in collaboration with patients, their carer's and other members of the Emergency Department Clinical team. This role also includes recruiting clients onto the program, conducting comprehensive assessments, coordinating all aspects of health and social care needs, monitoring achievement of client goals, facilitating intervention from other health care professionals and ensuring appropriate linkages with hospital and community services.
