Do work that matters, make a positive difference in your community Join a dynamic team in a successful and growing company Permanent full-time role working regular business hours The Commonwealth Home Support Programme (CHSP) is one of the programmes to help older people stay independent and in their homes and communities for longer. The CHSP provides entry-level home support for older people who need assistance to keep living independently. In this role, you will engage with our customers over the phone and at times face to face to conduct assessments, source domestic service assistance and other types of personal care, and coordinate programs and social outings.
You will oversee all customer communication to ensure the customer is heard and receives the quality personalised home and health care solutions as per their care plans and requirements to enhance their quality of life. You will also liaise with internal stakeholders to ensure customer's individual care plans and services are followed timely and accurately. You will be comfortable in managing customer issues and finding solutions to unexpected challenges that arise.
Key responsibilities include: Coordinating customer services in a fast-paced environment Conducting customer assessments and confirming appointments Managing customer database Multi-tasking competing stakeholder priorities at one time Developing and strengthening relationships with stakeholders Skills and experience required: CHSP experience is preferred or similar experience in Aged Care, Health Care OR an extensive understanding of the industry Previous customer service and administration experience is a must. Strong communication and influencing skills with a friendly and clear phone manner Positive problem-solver with a passion for helping people Attention to detail, strong time management skills and confident using computer systems Ideally, you will have experience working in the health, aged care or community services industry.
