Position Title: Primary Care Clinical Nurse Consultant - GP Support Line Employment Type: Full-time, Maximum-term for up to 3 years Hours per week : 38 Position Grade: Clinical Nurse Consultant 2 Salary: $117318.30 to $119650.68 per annum Enterprise Agreement: The Named NSW (Non-Declared) Affiliated Health Organisations' Nurses Enterprise Agreement 2017 Department Name: Cancer Stream Position Summary: A vacancy exists in the above unit for a highly motivated Clinical Nurse Consultant with appropriate knowledge and skills to join our friendly and supportive team.

The position will provide a Central and Eastern Sydney Public Health Network (CESPHN) region wide General Practice (GP) cancer support line to assist GP in accessing high quality advice and information from a cancer specialist, to enhance the management of cancer in primary care. The successful applicant will respond to enquiries from GP and will be expected to have a broad cancer and psycho-social, and cancer-related palliative care expertise and service knowledge of the CESPHN region.
The successful applicant will be expected to strengthen integration between our patient population and the General Practice (GP). The support line will respond to calls from GP's requesting support with cancer service navigation and cancer related enquiries such as: Survivorship care, Psychosocial care Palliative care and advance care planning Treatment and symptom management Service navigating advice Assistance with referral pathways and advice relating to the care of patients Our progressive and innovative team offers development opportunities for our staff and a caring environment for the delivery of high quality patient centred care.
